The phases of design are a constant refrain - they help place where in the problem-solving mode we are at. Applying human-centered design is most successful when it is clear exactly how we are to use our aha-moments, our explorations, to make a decision.

Decisions are made based on the information at hand - so I make sure teams build the capacity to recognize that humans are complex, biases are hard to circumvent by sheer intention, and the world is changing in ways hard to catch in existing dashboards. And that recognizing these things means you can design for success, both to your values as well as your bottom line.
